A Case Discussion on Integrating Structural and Cultural Nuances in Psychological Assessment 1.5 CEs
SPA E-Learning Center | 2025 SPA Convention
Abstract
Accurate diagnosis and differential diagnosis of neurodevelopmental and personality disorders is a complex process. This problem exists in part due to DSM-5-TR criteria being largely behaviorally based, as well as having significant overlap in symptom presentations as criteria (again, largely because they are based on observable behaviors, which can have different underlying etiologies). Further, symptoms may vary in their presentation across the lifespan and across different contexts. The goal of this case discussion is to examine the role of intersecting systems-level and identity-related factors in diagnostic decisions made when assessing individuals who are neurodivergent, have experienced trauma, and hold marginalized identities. This case discussion will have the following parts: (1) presentation of a sample client case and their assessment battery with score reports, including the MMPI, PAI, NEO-PI, and R-PAS, (2) interpretation of scores and diagnostic decisions with particular attention to structural and cultural-contextual nuance, (3) take-home recommendations for clinical assessment and guidelines for weighing the impacts of systemic oppression and nuances of intersecting identities, (4) group discussion period with the audience regarding personal approaches to these important issues in clinical assessment.

Goals & Objectives
- Describe relevant identity and systems-level factors in differential diagnosis of neurodevelopmental and personality disorders;
- Explain the importance of considering these factors when evaluating an individual’s clinical presentation;
- Interpret scores of major personality and clinical assessments with attention to context and intersectionality; and
- Apply presenter recommendations for contextualizing impacts of systems of oppression and nuances of intersecting identities in psychological assessment.
